Update Regarding Fees for Outside Paperwork Requests

Dear Parents ,

We hope this message finds you well. At Children's Medical Group, our goal is always to provide the highest quality care and service to your family.

As part of our ongoing efforts to manage administrative demands and ensure our team can continue to provide timely support, we are implementing a small processing fee for any paperwork requests not associated with a recent office visit.

This includes, but is not limited to, forms such as FMLA, disability paperwork, and other documentation that requires physician review and completion outside of a scheduled appointment.

Effective 6/1/2025, a $15.00 fee will be required for these forms.

 We understand that these documents are often important, and we are committed to completing them in a timely and accurate manner. However, due to the time and attention these requests require from our clinical staff, this fee helps support the resources needed to fulfill them properly.Thank you for your understanding and continued trust in our care.
