Please bring your insurance card to every visit. If you change insurance companies or join another employer’s plan, please be sure to let us know. If an insurance referral is necessary, please bring the completed form with you to your appointment.
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage. Please inform us of any changes to your insurance coverage.
We will fully attempt to help you receive full insurance benefits; however, you personally are responsible for your account. We recommend contacting us if your policy has not paid within a reasonable timeframe. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance company.
We are currently contracted with Aetna, Amerigroup, Blue Cross Blue Shield, Cigna, Bright Health, Oscar, Humana, United Healthcare, UHC Community Plan and Tricare Select and Prime (prime members must be in network area, call Tricare to confirm). We are NOT in-network with BCBS Network E, Cigna Surefit, Cigna Connect, or United Healthcare Navigate. If you are unsure that we are in-network with your plan, please contact your insurance company.
Currently, we are not accepting new patients over the age of three years with Amerigroup or UHC Community Medicaid plans. As of 1/1/2023 our office is no longer accepting any new patients with Bluecare/Tenncare Select Medicaid insurance, unless they are family members of current patients.
