New Patients Registration
Thank you for choosing Children's Medical Group. Our Patient Portal is designed to facilitate care by making your child's essential health information, patient forms, and other details accessible to you at any time. To register please call the office to ensure all of our communication information is correct and up to date. We will guide you through the registration process from there.
Existing Patient Registration
To connect an existing patient record to our new Patient Portal system, please submit the form below to request a registration PIN. The PIN can be sent via email or text message. Once you receive the PIN, you will have 24 hours to set up your account.
If you have multiple children in our office, you will need to request a separate PIN to register each one. However, once the registration process is complete for each child, you will be able to access all of their Patient Portal accounts through one login.
This system is new as of Summer 2020. If you registered for a Patient Portal account prior to this, you will need to register again. We apologize for any inconvenience this may cause.
